Changing Your Name on a Valid US Passport
United States Passport Name Change Requirements and Instructions
Note: If you are departing the U.S. within 4 business days, you must call us to ensure we have the capacity to process your passport application in time for your departure.
There are three steps to changing your name on a US passport:
Send us your Passport.
If you do not have your passport, you will need to apply for Replacing a Lost or Stolen Passport.
Original Name Change Documents
Send us the original or certified copy, plus one photocopy, of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned to you with your amended passport.
You will be required to submit two (2) identical passport photographs. Photographs must be taken by a passport photographer. Homemade photos are not acceptable and most often cause a delay in passport issuance.
We can prepare passport photographs for you at select A Briggs office locations nationwide. Review our Passport Photo Locations to see if there is one located near you.
If passport is GREATER than 1 year old: Click on the link at the bottom of this section (Section D) to create a DS-82 Application for Passport by Mail.
After completing the questions on the form, click the “Create Form” button at the bottom of the page. A completed DS-82 Application for Passport by Mail form will appear on the screen.
US Passport applicants may electronically submit their personal information and then print the completed form. This system generates a 2D bar code (required for all applications submitted by hand carry, the submission method used by A Briggs) on the printed form. This 2D barcode enables the information to be reproduced electronically. All information is captured on a secure U.S. Department of State server and is deleted once the form is printed. Please note the US Passport Office will not accept hand carry applications without the 2D barcode.
Print 2 copies of all pages of the DS-82 form, including the instructions, so that each page is printed only on one side of the paper. Do not print on both sides of any sheet of paper.
You must sign the completed DS-82 on the last page of the DS-82 form.

If Passport is LESS than 1 year old: Complete the DS-5504 US Passport Re- Application form.
(The State Department web site does not support the Safari, Opera or Chrome web browsers for passport form completion; Internet Explorer and Firefox should work fine, however.)
If Passport is GREATER than 1 year old: A check or money order made payable to "U.S. Department of State" for $170.
If passport is LESS than 1 year old: A check or money order made payable to "U.S. Department of State" for $60.
NOTE: If paying by check:
Please include your date of birth on the memo line of the check. Do not use starter checks for new bank accounts. Although it is recommended that your check have your name pre-printed on it, if submitting a business check or someone else's check, include your name, address and date of birth on the check. Money orders must contain your information on them.
Letter of Authorization
Submit 2 sets of letters of authorization. A letter of Authorization gives A Briggs the authority to submit your application and pick-up your passport when issued. We are only allowed to submit a certain number of passports each day, and the additional names on the additional authorization letters are for our sister companies under whose name we may submit your application in order to provide the fastest available service.
NOTE: Each copy MUST contain your original signature preferably in Blue Ink. Fax or photo copies of your signature are NOT acceptable - both letters must contain original signatures.
Proof of Departure
Submit 2 copies of your computer-generated itinerary or your ticket from an airline, travel agency, or online booking agency showing your departure from the U.S. FAX or photocopies are acceptable.
If you are unable to provide an itinerary please provide a letter from your company on letterhead. This letter should explain details of the trip and needs to be signed by someone other than the applicant.
If you are unable to provide a trip itinerary or company letter, we can still assist you without one however it will take a minimum of 11 business days.
Online Order Form and Payment
The Online Order Form must be completed. Please print two copies of the confirmation page. Include one copy in the A Briggs envelope and keep one copy for your records.
The Online Order Form will prompt you for payment method. You may pay the A Briggs fees online with a credit card or enclose a personal or business check or money order.
Processing times are business days and do not include shipping time. Keep this in mind when placing your order. Orders are filled based on departure date. For example, a traveler leaving in 10 days that selects 1 to 4 day service may experience fulfillment times of up to 6 days including shipping.
